For centuries, tuberculosis, which is a chronic infection caused by the bacillus Mycobacterium tuberculosis has remained a global health problem. The global burden of tuberculosis has increased, particularly in the Southern African region, mainly due to HIV, and inadequate health systems which has in turn given rise to emergent drug resistant tuberculosis (TB) strains. Bovine tuberculosis (BTB) has also emerged as a significant disease with the tendency for inter-species spread. The extent of interspecies BTB transmission both in urban and rural communities has not been adequately assessed. The phenomenon is of particular importance in rural communities where people share habitats with livestock and wildlife (particularly in areas near national parks and game reserves). Aerosol and oral intake are the major routes of transmission from diseased to healthy individuals, with health care workers often contracting infection nosocomially. Although TB control has increasingly been achieved in high-income countries, the disease, like other poverty-related infections, has continued to be a disaster in countries with low income economies. Transmission of infections occurs not only amongst humans but also between animals and humans (and occasionally vice versa) necessitating assessment of the extent of transmission at their interface. This review explores tuberculosis as a disease of humans which can cross-transmit between humans, livestock and wildlife. The review also addresses issues underlying the use of molecular biology, genetic sequencing and bioinformatics as t tools to understand the extent of inter-species cross-transmission of TB in a 'One Health' context.

Infectious diseases account for nearly 40% of the burden of human mortality and morbidity in low-income countries, of which 7% is attributable to zoonoses and 13% to recently emerging diseases from animals. One of the strategic approaches for effective surveillance, monitoring and control of infectious diseases compromising health in both humans and animals could be through a combination of multiple disciplines. The approach can be achieved through a joint effort from stakeholders comprising health professionals (medical and veterinary), social, economic, agricultural, environmental and other interested parties. With resource scarcity in terms of number of staff, skills and facility in low-income countries, participatory multi-sectoral and multidisciplinary approaches in limiting the burden of zoonotic diseases could be worthwhile. We review challenging issues that may limit the 'One Health' approach for infectious diseases surveillance in Tanzania with a focus on Health Policy and how best the human and animal health systems could be complemented or linked to suit the community in need for disease control under the theme's context.

Despite the apparent public health concern about Bovine tuberculosis (BTB) in Tanzania, little has been done regarding the zoonotic importance of the disease and raising awareness of the community to prevent the disease. Bovine tuberculosis is a potential zoonotic disease that can infect a variety of hosts, including humans. The presence of multiple hosts including wild animals, inefficient diagnostic techniques, absence of defined national controls and eradication programs could impede the control of bovine TB. In Tanzania, the diagnosis of Mycobacterium bovis in animals is mostly carried out by tuberculin skin testing, meat inspection in abattoirs and only rarely using bacteriological techniques. The estimated prevalence of BTB in animals in Tanzania varies and ranges across regions from 0.2% to 13.3%, which is likely to be an underestimate if not confirmed by bacteriology or molecular techniques. Mycobacterium bovis has been detected and isolated from different animal species and has been recovered in 10% of apparently healthy wildebeest that did not show lesions at post-mortem. The transmission of the disease from animals to humans can occur directly through the aerosol route and indirectly by consumption of raw milk. This poses an emerging disease threat in the current era of HIV confection in Tanzania and elsewhere. Mycobacterium bovis is one of the causative agents of human extra pulmonary tuberculosis. In Tanzania there was a significant increase (116.6%) of extrapulmonary cases reported between 1995 and 2009, suggesting the possibility of widespread M. bovis and Mycobacterium tuberculosis infection due to general rise of Human Immunodeficiency virus (HIV). This paper aims to review the potential health and economic impact of bovine tuberculosis and challenges to its control in order to safeguard human and animal population in Tanzania.

OBJECTIVE: To measure the costs and estimate the cost-effectiveness of the ProTEST package of tuberculosis/human immunodeficiency virus (TB/HIV) interventions in primary health care facilities in Cape Town, South Africa. METHODS: We collected annual cost data retrospectively using ingredients-based costing in three primary care facilities and estimated the cost per HIV infection averted and the cost per TB case prevented. FINDINGS: The range of costs per person for the ProTEST interventions in the three facilities were: US$ 7-11 for voluntary counselling and testing (VCT), US$ 81-166 for detecting a TB case, US$ 92-183 for completing isoniazid preventive therapy (IPT) and US$ 20-44 for completing six months of cotrimoxazole preventive therapy. The estimated cost per HIV infection averted by VCT was US$ 67-112. The cost per TB case prevented by VCT (through preventing HIV) was US$ 129-215, by intensified case finding was US$ 323-664 and by IPT was US$ 486-962. Sensitivity analysis showed that the use of chest X-rays for IPT screening decreases the cost-effectiveness of IPT in preventing TB cases by 36%. IPT screening with or without tuberculin purified protein derivative screening was almost equally cost-effective. CONCLUSION: We conclude that the ProTEST package is cost saving. Despite moderate adherence, linking prevention and care interventions for TB and HIV resulted in the estimated costs of preventing TB being less than previous estimates of costs of treating it. VCT was less expensive than previously reported in Africa.

OBJETIVO: Medir los costos y estimar la costoeficacia del paquete ProTEST de intervenciones contra la tuberculosis/virus de la inmunodeficiencia humana (tuberculosis/VIH) en establecimientos de atención primaria de Ciudad del Cabo, Sudáfrica. MÉTODOS: Reunimos retrospectivamente datos anuales sobre costos utilizando sistemas de determinación de los costos basados en componentes en tres establecimientos de atención primaria, para poder así estimar el costo asociado a cada infección por VIH evitada y el costo por caso de tuberculosis prevenido. RESULTADOS: Los intervalos de los costos por persona para las intervenciones de ProTEST en los tres centros fueron los siguientes: US$ 7 - 11 para el asesoramiento y pruebas voluntarias (APV), US$ 81 - 166 para la detección de un caso de tuberculosis, US$ 92 - 183 para el régimen completo de terapia preventiva con isoniazida (TPI) y US$ 20 - 44 para el tratamiento preventivo con cotrimoxazol durante seis meses. El costo estimado por infección por VIH evitada mediante APV fue de US$ 67 - 112. El costo por caso de tuberculosis prevenido mediante APV (gracias a la prevención de la infección por VIH) fue de US$ 129 - 215, mediante la búsqueda intensificada de casos, de US$ 323 - 664, y mediante TPI, de US$ 486 - 962. El análisis de sensibilidad demostró que el uso de radiografías torácicas en el cribado para TPI reduce en un 36% la costoeficacia de ésta como medio de prevención de los casos de tuberculosis. El cribado para TPI tuvo casi la misma costoeficacia con o sin cribado mediante derivados proteínicos purificados de la tuberculina. CONCLUSIÓN: Llegamos a la conclusión de que el paquete ProTEST permite ahorrar costos. Pese a la moderada observancia de los tratamientos, vinculando las intervenciones de prevención y atención para la tuberculosis y el VIH, los costos estimados para la prevención de la tuberculosis fueron inferiores a las estimaciones anteriores de los costos asociados al tratamiento de la misma. Las medidas de APV fueron menos costosas de lo que hasta ahora se había señalado en África.

OBJECTIF: Mesurer les coûts et évaluer le rapport coût/efficacité de l'ensemble d'interventions ProTEST contre la tuberculose et le virus de l'immunodéficience humaine (VIH) dans des établissements de soins de santé primaires de la ville du Cap, en Afrique du Sud. MÉTHODES: Des données relatives aux coûts annuels ont été recueillies rétrospectivement par évaluation des coûts à partir des éléments de cette intervention. Le coût par contamination par le VIH prévenue et celui par cas de TB évité ont ensuite été estimés. RÉSULTATS: Dans les trois établissements étudiés, le coût par personne des interventions ProTEST se situait dans les plages suivantes : US $ 7-11 pour la délivrance de conseils et le dépistage volontaire, US $ 81-166 pour la détection d'un cas de tuberculose, US $ 92-183 pour l'administration d'un traitement préventif par l'isoniazide et US $ 20-44 pour l'administration d'un traitement préventif de six mois par le cotrimoxazole. Le coût par contamination par le VIH évitée grâce aux conseils et au dépistage volontaire a été estimé à US $ 67-112. Le coût par cas de TB prévenu par ce même type d'intervention (prévention du VIH) était de US $ 129-215, par recherche intensive des cas de US $ 323-664 et par traitement préventif par l'isoniazide de US $ 486-962. Une analyse de sensibilité a mis en évidence que l'utilisation de la radiographie pulmonaire comme méthode de dépistage préliminaire à ce traitement permettait de prévenir les cas de TB dans une proportion de 36 %. Le dépistage préliminaire au traitement par l'isoniazide avec ou sans intradermoréaction par la fraction protéique purifiée de la tuberculine présentait une efficacité économique presque équivalente. CONCLUSION: L'article parvient à la conclusion que l'ensemble d'interventions ProTEST permet de réaliser des économies. En dépit d'une adhésion moyenne, le fait de lier les interventions à visées préventives à celles délivrant des soins contre la TB ou le VIH a entraîné une baisse du coût estimé de la prévention d'un cas de TB par rapport aux estimations antérieures du coût d'un traitement antituberculeux. L'étude indique un coût des interventions de conseil et de dépistage volontaire moins élevé que les chiffres rapportés antérieurement pour l'Afrique.

The development of WHO's DOTS strategy for the control of tuberculosis (TB) in 1995 led to the expansion, adaptation and improvement of operational research in this area. From being a patchwork of small-scale studies concerned with aspects of service delivery, TB operational research shifted to larger-scale, often multicountry projects that were also concerned with health policy and the needs of health systems. The results are now being put into practice by national TB control programmes. In 1998 an ad hoc committee identified the chief factors inhibiting the expansion of DOTS: lack of political will and commitment, poor financial support for TB control, poor organization and management of health services, inadequate human resources, irregular drug supplies, the HIV epidemic, and the rise of multidrug resistance. An analysis of current operational research on TB is presented on the basis of these constraints, and examples of successful projects are outlined in the article. We discuss the prerequisites for success, the shortcomings of this WHO- supported programme, and future challenges and needs.

El desarrollo de la estrategia DOTS de la OMS para el control de la tuberculosis en 1995 propició la expansión, adaptación y mejora de las investigaciones operativas en ese terreno. De ser un mosaico de estudios en pequeña escala centrados en aspectos de la prestación de servicios, las investigaciones operativas sobre la tuberculosis han pasado a ser proyectos en gran escala, a menudo multipaíses, interesados también por las políticas sanitarias y las necesidades de los sistemas de salud. Los programas nacionales de lucha contra la tuberculosis están llevando ya a la práctica los resultados conseguidos. En 1998 un comité especial identificó los principales factores que entorpecen la expansión de la DOTS: falta de voluntad y compromiso políticos, escaso apoyo financiero a la lucha contra la tuberculosis, mala organización y gestión de los servicios de salud, recursos humanos insuficientes, suministro irregular de medicamentos, epidemia de VIH y aumento de la polifarmacorresistencia. Se hace aquí un análisis de las actuales investigaciones operativas sobre la tuberculosis en relación con esas limitaciones, y se presentan sucintamente ejemplos de proyectos exitosos. Analizamos asimismo las condiciones necesarias para el éxito de esas iniciativas, las deficiencias de este programa apoyado por la OMS y los futuros retos y necesidades.

La mise en œuvre en 1995 de la stratégie DOTS de l'OMS pour lutter contre la tuberculose a conduit à élargir, adapter et améliorer la recherche opérationnelle dans ce domaine. Partant d'une mosaïque d'études d'envergure limitée axées sur les aspects de la prestation des services, la recherche opérationnelle a évolué au profit souvent de projets multipays de plus grande envergure, englobant aussi politique de santé et besoins des systèmes de santé. Les programmes nationaux de lutte contre la tuberculose tiennent actuellement compte des résultats obtenus. En 1998, un comité spécial a défini les principaux facteurs faisant obstacle à l'élargissement de la stratégie DOTS : absence de volonté politique et d'engagement, insuffisance des appuis financiers pour combattre la tuberculose, mauvaise organisation et gestion des services de santé, insuffisance des ressources humaines, approvisionnement en médicaments irrégulier, épidémie de VIH/SIDA et augmentation de la polypharmacorésistance. On trouvera dans le présent article une analyse de la recherche opérationnelle actuelle compte tenu de ces obstacles ainsi que des exemples de projets ayant abouti. Y sont également évoqués les préalables indispensables pour mener à bien un projet, les faiblesses du programme soutenu par l'OMS ainsi que les défis et les besoins futurs.

Human immunodeficiency virus (HIV) is fuelling the tuberculosis (TB) epidemic, particularly in sub-Saharan Africa. However, despite their close epidemiological links, the public health responses have largely been separate. WHO has set out a strategy to decrease the burden of HIV-related TB, comprising interventions against both TB and HIV. Voluntary counselling and testing (VCT) for HIV can link TB and HIV programme activities. The benefits of VCT for HIV to TB patients include referral for appropriate clinical care and support for those testing HIV-positive. Likewise, people attending a centre for VCT can benefit from TB screening: those found to be both HIV-positive and with active TB need referral for TB treatment; those without active TB should be offered TB preventive treatment with isoniazid. To explore how VCT for HIV can contribute to a more coherent response to TB, WHO is coordinating the ProTEST Initiative. The name "ProTEST" is derived from the Promotion of voluntary testing as an entry point for access to the core interventions of intensified TB case-finding and isoniazid preventive treatment. Other interventions may be added to provide finally a comprehensive range of HIV and TB prevention and care interventions. Under the ProTEST Initiative, pilot districts are establishing links between centres for VCT for HIV and TB prevention and care. This will pave the way for large-scale operationalization of the comprehensive range of interventions needed to control TB in settings with high HIV prevalence.

El VIH está atizando la epidemia de tuberculosis, sobre todo en el África subsahariana. Sin embargo, pese a su parentesco epidemiológico, las respuestas de salud pública contra las dos enfermedades han sido en gran medida independientes. Para disminuir la carga de VIH relacionada con la tuberculosis, la OMS ha formulado una estrategia que comprende intervenciones tanto contra ésta como contra el virus. El asesoramiento y las pruebas voluntarias (APV) para el VIH pueden constituir un puente entre las actividades de los programas dirigidos contra la tuberculosis y contra el VIH. Entre los beneficios del APV para los pacientes con tuberculosis cabe citar la derivación hacia servicios de atención clínica y apoyo apropiados para las personas VIH-positivas. Análogamente, las personas que acuden a un centro de APV pueden beneficiarse de las actividades de cribado de la tuberculosis: los pacientes que además de ser VIH-positivos sufren tuberculosis activa deben ser derivados para recibir tratamiento antituberculoso; y a aquellos sin tuberculosis activa se les debe administrar isoniazida como tratamiento preventivo contra la tuberculosis. A fin de determinar cómo puede el APV para el VIH contribuir a responder de forma más coherente a la tuberculosis, la OMS está coordinando la Iniciativa ProTEST, así bautizada por cuanto pretende promover las pruebas voluntarias como punto de entrada para acceder a las intervenciones básicas e intensificadas de detección de casos y tratamiento preventivo con isoniazida. Se pueden añadir otras intervenciones para proporcionar finalmente un conjunto integral de medidas de prevención y atención contra el VIH y la tuberculosis. En el marco de la Iniciativa ProTEST, diversos distritos piloto están estableciendo vínculos entre los centros de APV para el VIH y la prevención y atención ofrecidas contra la tuberculosis. Esto sentará las bases para aplicar a gran escala el amplio conjunto de intervenciones que exige el control de la tuberculosis en los entornos con alta prevalencia de infección por el VIH.

Le VIH intensifie l'épidémie de tuberculose, particulièrement en Afrique subsaharienne. Cependant, malgré leur relation épidémiologique étroite, ces deux infections donnent lieu à une réponse très différente de la part des services de santé publique. L'OMS a élaboré une stratégie visant à réduire la charge de la tuberculose liée au VIH et comprenant des interventions dirigées à la fois contre la tuberculose et contre le VIH. Le conseil et dépistage volontaire pour le VIH (VCT) permet de relier les activités des programmes de lutte contre le VIH et contre la tuberculose. L'intérêt du conseil et du dépistage du VIH chez les patients atteints de tuberculose réside dans la possibilité d'adresser les sujets VIH-positifs à un service spécialisé en vue d'un traitement et d'un soutien appropriés. De même, les patients qui viennent en consultation pour un VCT peuvent bénéficier d'un dépistage de la tuberculose. Les sujets à la fois VIH-positifs et atteints de tuberculose active doivent être orientés en vue d'un traitement antituberculeux ; ceux qui ne présentent pas de tuberculose active se verront proposer un traitement antituberculeux préventif par l'isoniazide. Afin de rechercher de quelle façon le conseil et dépistage volontaire pour le VIH peut contribuer à l'adoption d'une réponse plus cohérente face à la tuberculose, l'OMS assure la coordination de l'initiative ProTEST. Le nom de cette initiative renvoie à la promotion du dépistage volontaire comme point de départ pour l'accès aux interventions de base que sont la recherche intensifiée des cas de tuberculose et le traitement préventif par l'isoniazide. D'autres interventions pourront être ajoutées pour parvenir à tout un éventail d'interventions de prévention et de traitement du VIH et de la tuberculose. Dans le cadre de l'initiative ProTEST, des districts pilotes établissent des liens entre les centres de VCT pour le VIH et les centres de prévention et de traitement de la tuberculose. Cela ouvrira la voie à la mise en œuvre à grande échelle de toute la gamme d'interventions requise pour combattre la tuberculose dans les régions de forte prévalence du VIH.
